Most residential programs don’t offer trauma-specific therapies like EMDR and CPT. Astoria does — delivered by a masters-level co-occurring clinician who is both EMDR and CPT trained. If trauma is driving the addiction, we treat the trauma.

Our executive director Josh Davis is a retired Army veteran. Programming addresses the full military experience — before, during, and after service. TriWest accepted.
Equine Therapy. Weekly. Off-Site.
Every week, clients leave the facility for off-site equine-assisted therapy. It’s one of the most effective tools for trauma processing and emotional regulation — and one of the hardest to find at the residential level.

When residential ends, the next step doesn’t have to mean starting over with a new team. Our Milwaukie PHP/IOP program near Portland is built specifically to receive Astoria graduates — veteran-specialized, with sober living included.
Seven Days a Week. Not Five.
Programming runs seven days a week. Most facilities give clients unstructured weekend time — one of the highest-risk periods in early recovery. We don’t.

We work with methadone clients in coordination with local providers. MAT is available and managed by our medical team from admission.

PHP / IOP — Milwaukie (Portland Area)
Designed as a step-down from Astoria, the Milwaukie program is built to support residential graduates. The program follows an 8-week curriculum based on the 8 Dimensions of Wellness, with veteran-specialized case management. Sober living is available.

Before You Pack Your Bag
Can I bring my pet?
Astoria accepts service animals only, on a case-by-case basis. ESAs are not permitted. Veterans with service animals are reviewed individually before admission. All animals must have current documentation, shot records, and meet behavioral requirements.
What about my phone?
Phones are managed during your stay. You’ll have time to stay connected to the people who matter — but structure around phone use is part of what makes early recovery work.
What should I bring?
Pack for about 10 days of clothes — comfortable, layered for Oregon weather, with gym shoes. Detox rooms have 3 beds, residential rooms have 2. Parking is very limited so if you’re driving, plan accordingly. Shopping trips run Tuesdays for tobacco/nicotine and Fridays for personal items.
Will I actually leave the facility?
Yes, regularly. Every Wednesday afternoon is equine therapy off-site — clients work with horses guided by an instructor, no riding required. Saturday afternoons are dedicated outings: the beach, Fort Stevens, the Astoria Column, the Goonies House, bowling, maritime museum, Lewis and Clark trails. Monday, Tuesday, Thursday, and Friday afternoons include the local aquatic center — pool, hot tub, weights, treadmill.
Can my family visit?
Passes are available after 14 days in the facility. Family visits require advance request submitted by Wednesday community meeting. Family sessions available 1–2 times per week via Zoom, phone, or in person throughout your stay.
How long will I actually be here?
Detox averages 5–10 days commercial, 7–10 days TriWest, 5 days OHP. Residential averages 21 days commercial, 45 days TriWest, 30–45 days OHP. Most clients then step down to our Milwaukie PHP/IOP program near Portland — your case manager builds that plan with you before discharge.
